30 minute massage as part of a multi-component treatment plan following approval from WCB. MANDITORY: Paperwork must be obtained from therapist filled out and returned to their Paperwork must be submitted by therapist to WCB and approved BY WCB prior to booking ALL paperwork is available by both therapists upon request.
